This Letter of Authorization (LOA) is required on every insurance claim we handle. It authorizes us to:
- Communicate directly with your insurance carrier, adjuster, and mortgage company.
- Negotiate the documented scope and pricing of work performed.
- Receive insurance payments for the work as it progresses.
The form includes signature and initials fields. By submitting you are entering a binding agreement with Peridot Assist Construction & Design.
