Insurance Claim Authorization

I authorize Peridot Assist, LLC (a Georgia licensed general contracting company) to act as my General Contractor on the property identified above. I authorize Peridot Assist to inspect the property, document damage, prepare assessments and estimates, communicate with my insurance carrier and any inspectors, and proceed with insurance-approved repairs once the scope and pricing have been agreed upon in writing.

I understand that Peridot Assist is not a public adjuster and does not negotiate the terms of my insurance policy. Peridot Assist will document the actual scope of the work and prepare line-item estimates using industry-standard methods (including Xactimate or comparable estimating software).

I understand that any work performed beyond the insurance-approved scope is my responsibility unless separately agreed to in writing via a change order.

I authorize Peridot Assist, LLC to communicate directly with my insurance carrier, my insurance adjuster, my mortgage company, and any other party with a financial interest in this claim regarding the scope and pricing of the work.

I direct my insurance carrier and mortgage company to make payment for the work directly to Peridot Assist, LLC (or jointly to me and Peridot Assist, LLC) as the work progresses. I will sign endorsements and lien waivers as reasonably required to facilitate direct payment.

This authorization remains in effect until the claim is fully resolved and final payment has been made. I may revoke this authorization in writing at any time, subject to any work performed and amounts owed up to the date of revocation.
