Use this form to acknowledge that site documentation is complete and to formally authorize Peridot Assist to begin project execution under your signed Contractor Agreement.

This is a binding agreement. By signing, you confirm:
- Site documentation, scope, and pricing have been reviewed and approved.
- Peridot Assist may schedule trades and procure materials for your project.
- Work will proceed under the terms of the Contractor Agreement you previously signed.
Please read each section carefully and sign at the bottom. A countersigned PDF will be emailed to you for your records once received.
